I saw H2FSpawn's model for Ender 3 Pro linear rails for Y axis and created it again with some tuning in Fusion 360 from scratch, just with simmilar dimensions as a previous one. In comparison to previous model, I have added 2 ribs to make the bracket/adapter stiffer and also included a .step file from Fusion 360.
Beware that I didnt fully test the model yet, so feel free to come up with better remixes/modifications/comments. For materials, I recommend going with at least PETG, but ASA/ABS/Nylon is preferable due to temps close to hotbed. I have also extended the endstop. If its too long for you, print the one from H2FSpawns model, which is shorter.
Adaptor is made to supposedly mount on MGN12H carriage.
As for the threads/holes, these are:
On smaller side: 2x M5
On a bigger side: 2x M4 on sides, 4x diameter 3 (without threat)

Model might be 10 times larger than original size due to Fusion 360 doing Fusion things. Check before the print if it happens, then set scale to 1/10 or 0.1 depending on your slicer. I recommend adding more walls rather than infill for better rigidity.
