This is an X-axis carriage for "i3" style printers with two 8mm rods set 45mm apart. I use it on my Monoprice Maker Select (Wanhao Duplicator) but it could also fit a Prusa MK#.
It fits a Wham Bam Mutant v2 tool swap system. There's other Mutant carriages for i3 out there, but this one uses the adjustable belt tension system on Bliatun's printhead here:

The belt tensioner is far more robust at maintaining tension, easier to set, and easier on the hands then most belt retention designs I've seen.
The carriage body is designed with Ø4.8mm x3.9mm holes for M3 brass threaded inserts on both the front face (mating with Wham Bam Mutant) and two on the back for the belt clip piece.
I printed mine using Protopasta HTPLA-CF with heat treatment to ensure thermal stability. But you can pick any material that can survive hot end proximity without melting.
